For SEREN Connect facilitators

Please ask all young people who attend your SEREN Connect sessions to complete the feedback survey after each session. Their responses help us evaluate and improve the programme.

The same questionnaire is used for every session. Facilitators should ensure all participants have access to the survey and encourage them to complete it while the session is fresh in their minds.

GDPR and Anonymity

• The survey is anonymous. No personal data is collected.
• Responses are used solely to improve the SEREN Connect programme.
• If a participant chooses to include the name of their healthcare team at the end of the survey, they are consenting for their feedback to be shared with that team.
